Case Management

Medical Care Coordination staff are a team who can help clients who are newly diagnosed, who have been released from prison, or have fallen out of care. For home-bound clients needing day-to-day help, contact the Home Health Case Management staff.

What is Home Health Case Management?

C.A.R.E. provides nurse case management services for medically fragile clients and those needing more intensive medical support and attention to their psycho-social needs.  The Nursing Case Management team addresses the nursing, medical, and psychosocial issues and needs of the client. These services are provided by a registered nurse and a master’s level social worker.  A client is assigned a team based on the client's needs and available staff.

The home health team are responsible for developing, implementing, monitoring, and evaluating an agreed upon individualized plan of care for each client.  Advocacy is the primary role of case managers, and they do this by educating clients about resources and by referring them to appropriate community agencies as needed.  Through intervention and education, case managers seek to help each client maintain the highest degree of independence or autonomy as possible.

How to Access Home Health Case Management

Home Health Case Management has certain state and county eligibility guidelines and requires certain documents in order to enroll into the program.  Please contact the C.A.R.E. Program for more information:

What are Medical Care Coordinators?

Medical Care Coordinators work with more able-bodied clients with less intensive needs than our home health program. They can help clients access social services like programs to pay for your medical care, transportation, and food; and make referrals to drug treatment, mental health, and housing services.
